How to be referred to organisations that can help you connect to community groups, activities or services to improve and support your wellbeing.

Social prescribing is about connecting people to community groups, activities or services to improve and support their wellbeing. 

We work with a network of third sector organisations who are passionate about helping the people in our community. We connect these organisations together, making the network stronger and we support them with anything they may need. We also look at gaps in provision and work with support services to fulfil those gaps.

How to get access to help

You can either:

  • contact our community connectors who can signpost or refer you to services that can help
  • use our online directory and referral platform that lists local groups and support services
