What you need to provide in an Occupational Health referral

To help us to carry out an informed assessment, we need as much information as possible from you about each specific referral.

The referral must include the following information on the questionnaire along with an up to date copy of the employee’s job profile:

  • reason for referral
  • steps and actions you have already taken as the manager to assist the employee
  • questions you would like to be addressed in the Occupational Health Report

Insufficient information at the point of referral could ultimately affect the balance of the report.
